Fee Mail-in $45.00*
$100 late fee for renewal applications received after May 1 from competitive clubs.

Required for clubs conducting scheduled club practices:
Club practice insurance information & practice registration form (PDF)Each coach/volunteer must submit ONE of the following forms to be covered by the USATF liability insurance policy during club practices:
Youth Club: Coach/Volunteer Information Form $ Background Check Acknowledgement (PDF)
(for ALL coaches/volunteers who have direct contact with youth 17 & under during club practices/activities)- Adult Coach/Volunteer Registration Form (PDF)
(for coaches/volunteers who supervise club practices/activities where ALL participants are 18 years or older)
